This position will be based in the Rockville serving surrounding locations.
The Intake & Transiotion Coordinator will be responsible for receiving and processing all referrals intakes for the agency's Community Integration and Habilitation & Family Support Waiver programs, and all transitions from Supported Group Living to Waiver settings.
Coordinator will work directly with the individual(s), family/guardian(s), and Case Manager to plan and facilitate needed services during intake and/or transition.
Coordinator will develop a Positive Personal Profile (PPP), for each individual and provide education and resources regarding Competitive Integrated Employment and opportunities.
A high emphasis will be placed on providing employment education and development opportunities for the individual/family/guardian.
Coordinator will train CARS teams regarding PPP, and Individual Support Plan (ISP).
This position will coordinate with Service/Program Coordinators, and/or Employment Services to meet all service and support needs of the individual.
Bachelor's Degree (BA/BS) in Human Services, Education, Special Education and/or related field; number years of experience in the field may substitute for education.
Experience in case management or employment services, and/or a background in Social Services.
Experience with individuals with ID/DD is preferred.
OTHER REQUIREMENTS:
Must have a safe driving record and hold a valid driver's license Must have proof of auto insurance.
Must be capable of occasional transferring and lifting of clients - if needed Must be capable of providing services in any setting, i.
e.
facility, home, community, vehicle, etc.
Must obtain / maintain certification and be able to implement CPR & First Aid.
Must not have any evidence of abuse or fraud in any setting Must not have been convicted of a violent crime or crime related to a dependent population Must not have repeated or substantial violations of applicable laws and rules in operation of any type of residential, health or developmental program in the care of dependent persons.
